Risk Diagnosis

If you are proposing a risk diagnosis based on a problem-focused diagnosis or if you are proposing both a risk and a problem-focused diagnosis with the same focus, please use the same phrases for risk factors and related factors.

Please use:

• Phrasing according to the current content of the NANDA-I Classification, whenever possible. For example, use Substance misuse, not Substance abuse.
• MeSH terms to comprise part of the phrases, when available.

Please use:

• Phrasing according to the current content of the NANDA-I Classification, whenever possible. For example, use Neurocognitive disorders, not Dysfunctional neurocognition.
• MeSH terms to comprise part of the phrases, when available.

Please use:

• Phrasing according to the current content of the NANDA-I Classification, whenever possible. For example, use Men, not Male gender.
• MeSH terms to comprise part of the phrases, when available.

Interventions that can be initiated by the professional nurse that go beyond basic monitoring, referral to other professionals, compliance with organizational protocol, and/or that do not require orders from other health professionals. They are sanctioned by professional nurse practice acts or regulations.
